PARK RIDGE BASEBALL/SOFTBALL, INC.

WAIVER OF LIABILITY

I give my approval to allow this child to participate in any and all activities of Park Ridge Baseball/Softball, Inc. and its leagues, and agree that the child and parents will follow all league rules of play and conduct. I understand baseball and softball are contact sports which carry risks of collision, tripping and sliding as well as the risk of being struck by a ball or bat.  I further acknowledge that protective equipment worn by players does not prevent all injuries.

On behalf of my child and my spouse, I hereby waive, release, indemnify and hold harmless Park Ridge Baseball/Softball, Inc. and its leagues, the Park Ridge Recreation and Park District, the directors, officers, sponsors, supervisors, participants, managers, coaches and persons transporting my child to and from activities from any and all claims resulting from negligence or any other cause. 

I also understand that any registration fee will be refunded from Park Ridge Baseball/Softball, Inc. upon request made prior to the close of registration on April 1 of the upcoming spring/summer season  and requests made prior to the close of registration on  September 1 for the fall season.  No refunds will be made after the dates stated above. I also understand that a fee of $20.00 will be assessed for a late registration, if allowed, or in the event that a check is returned for any reason. 

I also understand registrants may not participate in any other concurrent baseball/softball program or league. Registrants may, however, attend baseball schools/camps, or play on school teams.

This Waiver of Liability also includes any injury which occurs at the Oakton Park batting cages which are owned and operated by the Park Ridge Recreation and Park District.
